Decision trail — RFCs, ADRs, and conventions for long-lived repos.

Governance Extras installs three governance skills: new-rfc (propose a cross-cutting change through an RFC with proposer/objector perspectives), new-adr (record an architectural decision with critique tracks), and update-conventions (evolve the CONVENTIONS.md with tracked changes). Ships RFC and ADR templates and seed READMEs. Repo-scope by default — governance artifacts belong to the project, not the engineer.
